If a pulley is misaligned, the cable enters and leaves the groove at an angle and rubs against both side flanges, producing wear on both sides of the groove. Excessive tension or a fairlead/cable problem tends to wear the groove base or chafe the cable, not symmetrically wear both groove flanges.
