What happens when you break the "chain of error"?

  • The accident still occurs.
  • The report gets submitted.
  • Accident does not happen.

Explanation

Because the error chain theory holds that an accident results from a sequence of linked events, breaking any single link interrupts that sequence so the accident does not occur. This is the whole purpose of identifying and removing contributory links before they combine to a final outcome.

suppose there are four points if u join them in a single straight line then it make straight chain ...those points are error and if all error happen in a manner where it makes straight chain ..then it will create accident or incident ..so if u break a any of the point(error ) from the line then we will be able to stop that accident or incident .
